FitQuest Fitness 1 Year Membership Terms:

By clicking on the I AGREE button below I am agreeing to the following terms:

• I understand that I am signing up for a 1 year contract and that I am responsible to pay $35 per month for 12 months.
• I understand that at the end of the initial 12 months my membership will continue on as a month-to-month membership without increase and that at that time I can cancel with 30 days written notice.
• I agree to allow automatic withdrawal of $35 from my checking/credit card account every month.
• I agree that payments shall begin to be deducted on this date each month beginning with the following month.

Please note: Your monthly payments will be made on the same date that you joined. (example, if you join on the 10th of the month, you pay make your payments on the 10th of every month).
